The United States health care system is undergoing a major transition from volume-based to value-based care. In California, health care pilots and initiatives offer financial incentives to clinics, hospitals, and health plans to accelerate this transition in order to better serve the state's nearly 14 million safety-net patients.

In order to optimize on the value of care rather than the volume of care, health plans and delivery systems are paying greater attention to factors beyond health care that influence a person's health, such as housing, food access, safety, education, and family stability. If health outcomes are to improve, safety net providers and health plans must to be able to understand the comprehensive needs and circumstances of their patients, and learn to coordinate with complex support systems to make sure those needs are met.
