The Fierce Urgency of Now: The John A. Hartford Foundation 2020 Annual Report

We have lived through a transformative year. Vaccinations are showing promise in controlling the COVID-19 pandemic that consumed much of 2020, and it is vitally important to take stock of what we have learned. From the harrowing experiences of frontline health care providers, to the pain felt by families who lost loved ones and were kept apart due to the virus, to the insidious structural racism and disparities continuing to harm our society, it has been a challenging time for all of us. Older adults and their families have disproportionately suffered the consequences, especially older adults of color and those living in nursing homes. As we reflect on the past year, our commitment to rapidly improving the care of older adults has only grown deeper and stronger.

Conversation Starters: Research Insights from Clinicans and Patients on Conversations about End-of-Life Care and Wishes

The John A. Hartford Foundation, Cambia Health Foundation, and California Health Care Foundation commissioned PerryUndem Research/ Communication to conduct focus groups among health care clinicians and patients on the topic of end-of-life care and wishes.This qualitative research comes on the heels of a national survey we conducted in spring 2016 among primary care providers and specialists who regularly see patients 65 and older. The national survey showed that nearly all physicians consider advance care planning conversations important, while, as of early March 2016, only a fraction had billed Medicare for such conversations using a new Medicare reimbursement code implemented in January 2016.The national survey identified key barriers to having these conversations, such as not having a formal assessment process in place, feeling uncertain about what to say in conversations with patients, feeling unsure when to have the conversations, and having difficulty dealing with family disagreements.The goal of the qualitative research was to explore experiences and ideas from both clinicians and patients around starting and having quality conversations about advance care planning and end of life. This qualitative research focused on various types of clinicians in contrast to the spring 2016 national survey, which only included primary care physicians and specialists.

Advancing Care Excellence for Seniors: Reflections on a John A. Hartford Foundation-funded Nursing Education Program

This brief will describe the work and assess the impact of the NLN Advancing Care Excellence for Seniors initiative, or ACE.S, which the John A. Hartford Foundation supported from 2009 to 2012. ACE.S was the first national effort to prepare students in all pre-licensure nursing programs, including associate degree programs, to deliver high-quality care to older adults in a variety of settings, by providing new educational opportunities, teaching tools, and other resources to help pre-licensure nursing faculty offer geriatric content and understanding to their students.
